【Vaccination Dates】 6 October 2026(Tuesday) - 25 December 2026(Friday) (Excluding Saturdays, Sundays, national holidays, and 7 December 2026) 【Vaccination Times】 ①Registration at 12:00 ②Registration at 12:15 *Please select either option ① or ② in the online reservation system.
【Fee】 3,500 yen (tax included) (Payment by cash or credit card) *If paying in cash, please have the exact amount so that there is no need to receive change.
【Eligible Individuals】 Ages 13 and older (Those under 18 require a parent or guardian's signature.)
【Items to Bring for Vaccination】 ・My Number card or Health insurance card ・Our clinic's patient card (if you have one) ・Pre-vaccination questionnaire (if you can print it out) *To ensure a smooth check-in process, please download the pre-vaccination questionnaire from the links below, fill it out, and bring it with you. 【Important Notes】 Please wear a mask as there are individuals at higher risk of severe illness from infectious diseases, such as the elderly and those with underlying medical conditions, Please note that the clinic does not provide masks, so you must bring your own.
